The following digital signature is required to confirm your understanding of your responsibilities regarding the new separate CVS drug card process.

IMPORTANT NOTICE FOR SPECIAL CIRCUMSTANCES:
1. SPECIALTY DRUG USERS- You must fill through CVS MAIL ORDER HELP DESK-Retail fills anywhere else are not allowed. You must call 1-800-237-2767 immediately to initiate your order. See custom instructions about any formulary issues when you print your temporary CVS card.
2. Current Mail Order Users- You need to call 833 956 1787 to register your mail order through CVS mail order. Health Alliance current mail order does not auto transfer. Safest option is to early fill through your old card if you are needing mail order drugs before Jan 7 as some setup time is required to configure your order at CVS going forward.
3. Retail 90 Day supplies must be filled at CVS. This saves employees and their Districts on future premium increase as it is the best value for maintenance drugs picked up at a retail pharmacy
4. Regular 30 day fills- Starting January 1, 2021-You can go to any in network pharmacy and have them update your Prescription Plan with your Temp Card Info.
NEXT ACTION:Please verify the covered individuals below, click to confirm and pin sign before printing a temporary copy of your new CVS card located at the bottom of the Household Member navigation list.
